Permission Agreements/ Acuerdos de Permisos

MEDICAL RELEASE
I give permission for the Staff, Physicians or EMT’s to administration First Aid to the below-named child and to those transporting my child to and from the Religious Education Program, Outing, Service Project or Event as their judgment deems advisable and to make the necessary referrals to qualified physicians for the treatment of illness or accidents.  I understand that I will be notified in the event of any serious illness or accident.  In case of an emergency, I understand that every effort will be made to contact the Parents/Guardians and in case they are not able to be reached, I hereby give permission to the St. Francis Staff, Physicians or EMT’s to make decisions to secure proper medical treatment.

PERMISSO MEDICO
Doy permiso para que el personal, médicos o EMT puedan administrar primeros auxilios para el niño mencionado anteriormente y para aquellos que transportan a mi hijo/a hacia y desde el programa de educación religiosa, evento, o proyecto de servicio según su juicio aconsejable y de hacer las referencias necesarias a médicos calificados para el tratamiento de enfermedades o accidentes. Entiendo que se me notificará en caso de enfermedad grave o accidente. En caso de una emergencia, entiendo que se hará todo lo posible para contactar a los padres y en caso de que no puedan ser alcanzados, por la presente doy permiso al personal de San Francis, médicos o EMT para tomar decisiones para asegurar tratamiento médico adecuado.

PHOTOGRAPHY RELEASE
I give permission for my child to be photographed at St. Francis Catholic Church events or celebrations. These photographs will be used for the Parish Bulletin, Website, our Facebook Page, App, newspapers, presentations or brochures. Photographs will only be printed, published or posted by the Communications Specialists, DRE, designated Parish Staff, or Pastor.

PERMISO DE FOTOGRAFIA
Doy permiso durante el año 2023-2024 para que mi hijo sea fotografiado en eventos o celebraciones de la Iglesia Católica de San Francis. Estas fotografías se utilizarán para el Boletín de la Parroquia, la página web, nuestra página de Facebook, la aplicación, los periódicos, y las presentaciones o los folletos.

Virtus: Teaching Safety-Empowering God's Children
This is a sexual abuse prevention program provided to us by the Archdiocese of Atlanta and is part of our ongoing effort to help create and maintain safe environments and to protect children and youth from sexual abuse. This program will be presented to enrolled students during the year. As the primary educator of your child, you have the rights to opt your child out of participating I the program. If you determine that you DO NOT want your child to participate, please initial below.

Este es un programa de prevención de abuso sexual proporcionado por la Arquidiócesis de Atlanta, es parte de nuestro esfuerzo continuo para ayudar a crear y mantener entornos seguros y para proteger del abuso sexual a los niños y jóvenes. Este programa será presentado a los estudiantes durante el curso escolar. Como educador principal de su hijo, usted tiene derecho a optar por que su hijo no participe en el programa. Si determina que NO desea que su hijo participe, por favor complete inicia debajo.
 

PERMISSION TO CONTACT YOUTH
St. Francis of Assisi Catholic Church follows the Archdiocese of Atlanta's Social Media Policy and Guidelines for the Use of Social Networking Sites with Minors for contacting youth via social media. We may also use text messages and email to contact youth. Per this policy and guidelines, parents must be made aware of how social media and electronic communications are being used. Parents must be told how to access the sites, and be given the opportunity to be copied on all material sent to their children.

Permission to contact youth for internal or external communications will be for one year via social media, email, or text. Parent can request the same communications provided to youth, and understands that it does not have to be via the same technology (for example, if children receive a reminder via Instagram, parents can receive it in a printed form or by an email list).

PERMISO PARE CONTACTAR A LA JUVENTUD
St. Fracnis of Assisi Catholic Church sigue las politicas de redes sociales y directrices para el uso de sitios de Internet donde se interactual socialmente con menores establecidas por la Aquidiocesis de Atlanta para ponerse en contacto con jovenes a traves de las redes socials. Tambien podemos utilizar mensajes de texto y correo electronico para contactar a los jovenes. Segun esta politica y directrices, los padres deben tener conocimiento sobre la forma en la que se estan utilizando las comunicaciones electronicas y las redes sociales. Los padres deben recibir informacion para acceder a los sitios de Internet, y tener la oportunidad de ser copiados en todo tipo de material enviado a sus hijos.

El permiso para contactara a la juventud en comunicaciones internas o externas sera por un año a traves de redes sociales, correo electronico o texto. Los padres pueden solicitar acceder a las mismas comunicaciones que recibe el joven, y que no siempre tiene que ser a traves de la misma tecnologia (por ejemplo, si los jovenes reciben un recordatorio a traves de Instagram, los padres puden recibirlo en forma impresa o como parte de una lista de distribucion de correo electronico).
